'Manchester : Disaster on Runway 24'; An engine fire on take-off spread to the fuselage of a British Airtours 737, killing 55. The 83 survivors all escaped in the first 2 minutes -- 'Potomac : the last flight of Palm 90'' engine icing caused Palm 90 to crash on take-off striking a bridge and coming to rest in the freezing Potomac River. Only 5 survived from 79 passengers and crew. -- 'Herald of Free Enterprise : Zeebrugge'; When the Herald of Free Enterpirse left Zeebrugge harbour, the bow doors were left open. The ferry capsized in 45 seconds, killing 193 passengers and crew
